Note Change ====> Early Class at 5:30 to 7:00 p.m. on Tuesdays and 1:30 to 3 p.m. on Saturdays
Later Class at 7:00 to 8:30 p.m. on Tuesdays and 3:30 to 5 p.m. on Saturdays

I recognize that I am registering on a first come, first served basis, the above student for:
1. the Elementary/Middle School Level I 6/7th Grade Early (5:30 to 7:00 p.m.& 1:30 to 3 p.m. on Saturdays) Class ______
2. the Elementary/Middle School Level I 6/7th Grade Later (7 to 8:30 p.m.& 3:30 to 5 p.m.)______ Class Full
The course requires close to equal numbers of boys and girls - 36 minimum/ 40 maximum.Payment in Full or Installments. $125 ($20 nonrefundable) holds slot. Total Cost $325 per student. $100 due upon receipt class confirmation and third final payment of $100 by 15 September. No refunds after 15 September unless case by case exceptional reason (like student just broke leg). Includes all lessons, the Level I Handbook, New Table Manners TrainingKnife and Fork Set, and Dinner-Dance (Three course Restaurant Meal, Tax & Tip, prizes, etc.).

You may register by email: bmanerly@hers.com to include all Registration form data as an attachment or embedded. Hardcopy form and check for minimum of $125 must be received within 5-7 days to keep slot.
